Assessment Officer

This is an exciting opportunity within Adult care to work as part of a multi-disciplinary team setting up personal budgets and direct payments to vulnerable adults within the community.

The role involves working with individuals to build an appropriate support plan in order that their identified outcomes, enabling them to remain at home, can be realised within budgetary constraints. This may involve the setting up of the delivery of care with either statutory or voluntary organisations or the provision of basic equipment in order to ensure their safety within their own home.This role is demanding and busy and good organisational and IT skills are important in ensuring the timely delivery of identified interventions.

The ability to quickly build relationships with service users, their families and carers is required: together with the ability to work in collaboration with other disciplines and external agencies.
