We are offering an exiting and unusual opportunity within the Access and Hospital Support Team. This is a new and developing team we have a wide remit including working with adults with physical disabilities to increase their independence and allow them to remain in their own home. We work closely with the hospital, community health Services, voluntary sector, independent providers and other Adult Social Services Teams
We are a multi disciplinary, joint Health and Social care team that is friendly and professional. We are working closely with the Acute Trusts to facilitate discharge and are developing close links with our partners within Home from Hospital.
The successful candidate should have established community experience and a good understanding of a wide range of services available throughout North Somerset. You would be working along side professionals in the team including Social Workers, Occupational Therapists, Physiotherapists and Nurses. An understanding of the Assessment Process, Personalisation and community Services (Voluntary, independent, Health and Social Care) would be an asset. You would be expected to keep accurate records and computer skills are essential.
