The East Surrey Hospital Health and Social Care Team is part of an integrated service providing assessment and rehabilitaion services to those needing assistance on discharge from hospital or for prevention of admission.
Role Description:
As part of a multidisciplinary team you will be responsible for managing your own caseload and for working holistically in providing assessment and arranging services for a wide range of adults. You will ensure that the social care needs of individuals are met by working with service users, carers and other agencies to deliver person centred, flexible and innovative services which improve quality of life. Based in a hospital setting you will work closely with your Surrey Primary Care Trust colleagues and other health partners. The service receives referrals from both community and hospital, which provides a varied and interesting workload. Excellent training opportunities are available to the successful candidate.
You will hold a qualification in Social Work, Occupational Therapy or be a Registered Nurse and have work experience in related areas. Knowledge and/or experience of assessing and care planning and an understanding of current legislation are essential. You will require IT skills and have good communication and numericy skills. You should be a self motivated and flexible individual who is able to liaise with people at all levels, and be keen to promote best practice. Travel will be required as part of this post.
An enhanced Criminal Records Bureau disclosure and adults barred list check will be required for this role.
