Role Description:
You will be working as part of a multi-disciplinary team consisting of physiotherapy, nursing, occupational therapy, rehabilitation assistants, and social care staff. The role will involve being responsible for all aspects of the care management process, safeguarding vulnerable adults, focussing on assessing and care planning with service users, their carers and families to ensure person-centred, flexible and innovative outcomes with a focus on re-ablement.

You will be also be working closely with the care managers and social care staff in our long term team.

In particular, you will manage a complex caseload, set the highest standards of professional practice and supervise junior colleagues, therefore every day will bring new challenges.

As a registered and qualified social worker, occupational therapist or registered nurse, you will bring sound practical experience and a real passion for making a difference. Familiarity with all aspects of social care, including supporting hospital discharges, anti-discriminatory practice, relevant legislation and staff development is essential, and you will share our deep commitment to enabling people to remain at home.
