Main Purpose Of Job
The post holder will have full responsibility for managing and assessing a caseload of clients. A key asect of this role will be to evaluate, negotiate and co-ordinate the application of resources and monitor the outcomes to meet the needs of the individuals concerned, in order to maintain or adapt to a more normal or safer lifestyle.
To negotiate for, commission and monitor appropriate resources to meet individual service plans within allocated budgets and to provide information, advice, counselling, support and guidance to both service users and their carers.
Further responsibility will be to provide direct social work intervention, where appropriate, as required by the care plan.
