Community Support Services require a Cobham Link Co-ordinator

Promoting independence, choice and control is at the heart of this post. This is an exciting opportunity to work with people in a person-centred way, proactively seeking to maximise their independence. If you want to make a difference to peoples’ lives – enabling them to enhance their wellbeing and independence, giving them choice and control over the support and care they receive – this may the job for you!

The post will be based at the Cobham Centre for the Community and will be responsible for helping people with learning disabilities to achieve their ambitions and Support Plan outcomes This will incorporate social education, self-care, independent living skills and employment opportunities.

Building on the principles of Self Directed Support, Cobham Link aims to:

  • Use the Cobham Centre as a base for activities for people with learning disabilities, providing a link to the local community
  • Increase local opportunities for people to volunteer, learn new skills and be active
  • Increase social opportunities for local people with learning disabilities, both during and outside of service hours
  • Increase the chances for inclusion and for people to get to know local people and to find friends and mentors

You will work in partnership with individuals, their carers, families, staff and service providers throughout the year.

Successful applicants will be skilled at communicating and working with people with learning disabilities and have experience of working directly with people with social care needs.

You will be confident and competent in working with people with a variety of physical and cognitive impairments. You will have skills in assessment, Project development and have the ability to work professionally in a multi-disciplinary / multi-agency context.
