Band 5 Community Learning Disability Nurse (Watford & Three Rivers Adult Disability Team)
Based in Hemel Hempstead

As a registered learning disability nurse working with people with learning disabilities, you will work to ensure that people with learning disabilities receive equitable healthcare from generic health services. You will hold a caseload supporting individuals, their families and carers to access the support they need to meet primary and secondary health care needs. You will be working in a team of social workers, nurses and OTs and liaising with local GPs, psychiatrists, psychologists and other nurses to deliver a service to people in the Watford & Three Rivers districts.

As part of an award winning Community Learning Disability Nursing service, you will support the development of partnerships with Primary Care Trusts, acute hospitals, podiatry, dentistry, optometry and other key health providers in Hertfordshire. You will support organisations to identify training needs and deliver training programmes whilst promoting equality in health for adults with learning disabilities.

We would welcome applications from newly qualified nurses and can offer a Preceptorship programme and access to a wide range of training to support your professional development.

Our service is currently in a period of transition with the joining together, in April 2017, of services for people with physical disabilities with those for people with learning disabilities. We are looking for someone who would be able to use this opportunity as part of their professional development and can support people and their carers to be active members of their communities.

The role will involve travelling around the area it is essential that you have use of a care. A lease car scheme is available for successful candidates.
