Frail, Elderly and Carer Co-ordinator – Part Time

Would you be able to develop support services and activity provision for local frail and elderly residents and their carers?

We are looking for someone who can liaise with internal and external partners that would have responsibility for frail and elderly residents to highlight gaps in services and identify opportunities for lifestyle and screening services, appropriate exercise schemes and support to isolated adults within independent living accommodation.

We are looking for someone with enthusiasm, passion and drive to ensure local service provision is developed in line with local need. The successful applicant will need to plan, organise and deliver efficiently and be able to work to deadlines.

Educated to A level standard, or equivalent, you will require excellent communication skills – both written and verbal, experience of Microsoft Office packages – especially Word, Excel and Powerpoint. A knowledge and understanding of the role of NHS bodies, as well as other statutory independent and voluntary organisations, is essential as well as the planning, implementation and management of projects.

Experience of local adult service provision and support services for the frail and elderly would be an advantage. You will require a suitable vehicle and a full clean driving licence. This post will require some evening and week-end working.

Satisfactory clearance of Criminal Records Bureau check will be a condition of employment as the post involves regular contact with vulnerable and elderly people.
