The Marie Curie Nurse – Discharge is responsible for actively supporting discharge and care at home as a viable option for end of life patients and their families.

This role involves linking and collaborating with other health and social care providers in primary, secondary, independent and voluntary care, to promote communication & seamless care and support for patients and their carers.
