Location:Based at Castlewood, Clevedon 

We are offering an exiting and unusual opportunity within the Access and Hospital Support Team. This is a new and developing team we have a wide remit including working with adults with physical disabilities to increase their independence and allow them to remain in their own home. We work closely with the hospital, community health Services and other Adult Social Services Teams

We are a multi disciplinary, joint Health and Social care team that is friendly and professional. We provide a service to give people the opportunity and choice to become more independent within the community.

The successful candidate should have established community experience and a good understanding of a variety of conditions that affect peoples function and ability. You would be working along side professionals in the team including Social Workers, Occupational Therapists, Physiotherapists and Nurses. An understanding of personalisation and community services (both Health and Social Care) would be an asset. You would be expected to keep accurate records and computer skills are essential.
