ADULT SOCIAL SERVICES

The Physical and Sensory Impairment Team provides care management and re-enablement support to adults with physical disabilities; long-term conditions; HIV/AIDS; asylum seekers and sensory impairments and carers. Many of our service users have needs which are several and complex.

The team is committed to outcome focused care and ensuring that service users are able to reach their maximum level of independence.

Several new opportunities have arisen in this exciting and innovative team which is making positive changes to the lives of our service users and their carers.

Re-enablement Team

You will be a qualified occupational therapist with experience of working closely with service users with complex medical and social needs, mainly living in their own homes.

Devising and delivering treatment plans with a goal oriented and outcomes focus, you will have knowledge of relevant legislation, equipment and services to maximise the independence of service users.

The ability to travel around the borough is essential.
