REHABILITATION CARE CO-ORDINATOR

Hours:18.5 hours per week (Monday/Tuesday/Wednesday) 

Working pattern:Part Time, Permanent position 

Location:Cheltenham & Tewkesbury Intermediate Care Team, Edinburgh House, Coronation Square, Cheltenham, Glos.

Intermediate Care in Cheltenham is developing and growing. We are looking to appoint an additional Care Co-ordinator to work alongside two existing staff.

This is an exciting opportunity to join a dynamic, multi-disciplinary team. The successful applicant will manage a team of Rehabilitation Assistants and liaise with clinicians within the team and other agencies to maximise service users potential. Ideally you will have a background in health or social care and experience of man management.
