you. The Older People’s Community Social Work Team are looking for Senior Social Work Practitioners to join us in providing an Assessment & Care Management Service for people over 65.

As a Senior Practitioner, you will be required to provide supervision to qualified and unqualified care management staff, undertake complex case management and contribute to the management of the team via the allocation of resources and monitor service delivery.

For this post you will be expected to hold a social work qualification, registration with the General Social Care Council – a minimum of three years post qualifying experience together with knowledge and experience of the provision of social care services and a commitment to achieving good outcomes for Older People and their carers.
