The Reablement team is a well established group of professionals who work actively to:
•Facilitate safe, effective hospital discharge
•Prevent hospital and care home admission
•Maximising independence choice and control for Older people and people with disabilities in their own homes.

The talented staff work closely with service users and their families to co- achievable skills gain programme to maximise the service user’s retained abilities to improve and sustain a satisfactory quality of life. For those who need more intensive care, careful planning for hospital discharges or moves into more supported environment takes place with service users and their families.

We are looking for talented, experienced professionals with tenacity, drive and enthusiasm to join our high performing team and deliver top quality support to our vulnerable population.

The Senior Social Worker in Reablement has a key role in ensuring that delayed discharges are kept to a minimum and liaising with all hospitals to agree delays. In addition they have responsibility for ensuing excellent quality professional social work practice amongst the social workers in the team. A keen eye on performance and a commitment to continuous improvement are attributes required for this role.
