Come and join our integrated Learning Disability service and help us to make positive differences in the lives of people with a learning disability and their families in North Somerset. Professionals, users and carers acknowledge the Community Team for People with Learning Disabilities to be a successful integrated multi-disciplinary team. We provide excellent training opportunities, regular supervision and appraisal. We encourage staff to contribute to the development of the team and of services in North Somerset for people with a learning disability and their carers.

An exciting opportunity has arisen in the team for another transitions social worker. You will have the opportunity to work closely alongside the existing transitions social worker, children’s services, education and Learning Partnership West in planning and setting up services for young adults with complex additional needs making the transition into adulthood. You will be a ‘link worker’ with children from 16-18, becoming the allocated social worker when they turn 18 and will work closely with the individual in a person centred way, with other health and social care colleagues. Knowledge of setting up personal budgets and supported living options in the context of Valuing People Now, will be an advantage. You will need to be creative and dynamic in your thinking when setting up packages of care whilst ensuring “Best Value” in this current financial climate.
