We are looking to consolidate our Intermediate Care service by recruiting permanently to our 2 posts for Community Occupational Therapists.

This is an opportunity to work in a fast moving and enthusiastic multi-disciplinary team to really make a difference to long-term outcomes for patients.

We offer intensive rehabilitation programmes in bed-based settings and in patients’ own homes. Also we offer a rapid response service to avoid hospital admission to those who need urgent multi-disciplinary intervention but do not need in-patient care. The posts rotate between the settings of our service.

Staff receive individual, profession-based supervision as well as support from colleagues within the team, across the disciplines of nursing, physiotherapy and social work. There is support for training and continuous professional development.
