This is an exciting opportunity to work at the forefront of health improvement with key agencies to reduce health inequalities in Waltham Forest. It is a partnership post with Waltham Forest PCT, in which you will be pro-active in engaging residents living in targeted neighbourhoods to address the major risk factors for coronary heart disease and cancer for those most at risk. You will undertake health assessments, offering one to one support and tailored interventions to empower people to make lifestyle changes through weight management, uptake of exercise and better management of health conditions. This role will require you to develop relationships with local health practitioners and others delivering health services, in order to develop a network of client focused support at neighbourhood level.
