We are recruiting Care Workers who are committed to promoting and maintaining independence, choice and control to frail older people and adults with physical disabilities in their own homes.
The team provides an assessment and reablement service for up to six weeks, working with people by assisting and encouraging them to manage all aspects of their daily living. If ongoing care and support at home is required we facilitate a handover to an independent long-term provider.
We are looking for experienced Care Workers ideally with NVQ Level 2/3 in Health and Social Care who are keen to work in an innovative service, focusing on providing short term input enabling people to live at home avoiding placement into long-term care.
Staff currently work across the day in 2 bands, 7:30am to 2:30pm and 2:00pm to 10:00pm flexibly over a seven day period. Vacancies exist in the Woking area for full- time, part-time and bank staff.
Independent Living Unit
We also staff an independent living unit for adults between the ages of 18 – 60 with physical disabilities in the Woking area. The purpose of this service is to enable service users to retain and regain their daily living skills to their maximum potential, enabling them to live independently in the community.
Night Service – This is a Pilot Scheme initially for six months with the expectation of continuing long-term. Staff will be working alongside the existing night services provided by the NHS Community Night Nurses to develop an Integrated Night Service across the Spelthorne (Staines & Sunbury) West Elmbridge (Walton & Weybridge), Runnymede (Egham & Chertsey) and Woking & Byfleet area.

Night Community Care Assistants Assessment & Re-ablement team is an extension of our day services and will be based at St. Peters Hospital Chertsey with the NHS Community Night Nurses and will work alongside them from 10:00pm to 8:00am.
A full driving licence and own transport is essential for all the above vacancies.
An enhanced Criminal Records Bureau disclosure and POVA check will be required for this role.
