Joined up Care Derbyshire is seeking expressions of interest from staff currently working in Joined Up Care Derbyshire organisations involved with the Care Transfer Hub and who are interested in taking on the exciting opportunities listed below (JDs attached). This includes colleagues working for Derbyshire County Council. The system is now moving to further develop the team which forms an important component under the wider Community Transformation programme.

The Care Transfer Hub has been a long-standing development in JUCD, in line with the national expectations from NHSE. Put simply, the Care Transfer Hub is a multiple professional team, working as a single team, to ensure as many people as possible go home in a timely way after a stay in an acute hospital. We are at an exciting point in that we now want to move forward with a few key roles that will help us bring the Care Transfer Hub concept into a reality.

The posts outlined below are being offered initially for 6 months so we can build in a formal review of the Care Transfer Hub’s development, aligned to the Community Transformation programme.

We want to fill these posts from the JUCD partners involved in the hub to demonstrate our shared commitment, and to help us engage and bring teams and colleagues on the journey with us.

These roles are for people who are passionate about driving system change, capable at leading and being involved in service delivery across organisations, and who are motivated to getting more people home from hospital. The opportunity for secondment into these temporary posts would need to be agreed internally through operational line management and the details of how to apply are below.

Care Transfer Hub Operational Lead

  • The role is being advertised at salary range £64,445 - £74,896 dependent upon experience.
  • This position is ideal for someone who is passionate about driving system change, leading service delivery, and enhancing operational processes,and who is ready to make an immediate contribution.
  • We’re looking for someone who can commence as soon as possible, and who brings strong skills in coordination, stakeholder engagement and operational leadership.

  • The role is being advertised at salary range £31,049 – 37,796 dependent upon experience.
  • This position is ideal for someone with a keen eye for data and skilful in compiling reports.
  • We’re looking for someone who is open to being challenged and capable of shaping the administration processes to support accurate data collection.

  • The role is being advertised at salary range £24,937 – 26,598 dependent upon experience.
  • This position is ideal for someone who thrives in a fast-paced environment and enjoys supporting multiple administrative processes within a multi-disciplinary team.
  • We’re looking for someone who is organised, an excellent communicator and willing to learn to develop their skills across multiple IT platforms.
