We're looking for social work England registered Social Workers to join us.

We are currently unable to accommodate newly qualified social workers, however you are welcome to apply to be notified of when the opportunities become available.

We have opportunities within our adult disability, older people and hospital teams across Hertfordshire.

Older Peoples Team East

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.

Hospital Integrated Discharge Team

£38,220 - £41,771 per annum, plus £1,911 - £2,088 market enhancement pay plus 5% operational allowance per year

These enhancements are non-contractual payments, subject to a 3 yearly review

You'll ensure safe and timely discharges from hospital by:

  • Assessing the needs of patients and carers
  • Arranging appropriate care packages
  • Liaising with professionals and agencies
  • Supporting person-centred discharge planning

37 hours per week, 25 hours might also be considered

Post Hospital Review team
Herts Valleys Out of County Hospital Team
37 hours per week, 25 hours might also be considered

We are unable to accept AYSE Social Workers at this time

We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team

About you

Essential:
• Social Work degree
• Registration with Social Work England
• Driving licence

Driving required
You’ll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations.

English Fluency
The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).     • £38,220 - £41,771 per year
