
Best Start Inclusion Practitioner

Hartlepool Best Start Family Hubs are recruiting enthusiastic, committed practitioners to be part of our Best Start Inclusion offer, supporting children aged 0-5 with emerging additional needs, and their families.
You will deliver flexible support on a one to one and group basis in Best Start Family Hubs, community settings, early years provision and family homes to identify needs early and improve developmental outcomes for children. Practitioners will use evidenced based screening and observation tools to assess development and inform support (eg ASQ, WellComm, ELIM)
As a Best Start Inclusion Practitioner, you will provide practical advice and empower families to navigate and access appropriate services, including for complex needs, working alongside family hub navigators and signposting to trusted services. You will support effective transition from early years to school for children with SEND, additional or emerging needs.
