The CYPN are a small and supportive team who work across the county. They work to ensure children and young people (CYP) aged 5-19 years (25 SEND), and their families can access support on a range of topics with a strong focus on public health.

The team supports children and young people (CYP) whose health and/or medical needs affect their access to education. They work collaboratively with colleagues across Lincolnshire County Council, including the Pupil Reintegration Team (PRT), Children's Services (Children’s Centres, Early Help, Social Care, Family Hubs, Inclusion and Attendance), and Children's Health services such as Health Visiting Teams. They also liaise with external partners, including education providers and GPs, to promote inclusion, remove barriers to learning, improve outcomes, and reduce health inequalities for children, young people, and their families.

The role

A key aspect of the role is undertaking holistic health assessments for CYP to identify their health, wellbeing and developmental needs. Using professional judgement, you will work with CYP, families and partner agencies to develop appropriate support plans, provide advice and guidance, and coordinate interventions that help CYP achieve the best possible outcomes, including accessing and engaging in education. You will ensure support is tailored to the individual needs of each child and family. There is a strong emphasis on early support and prevention for CYP therefore the team complete Early Help Assessments to ensure that all the appropriate agencies are there to support a CYP and their families. This may also include starting a Team Around the Family (TAF) meeting and being lead professional.

The team offers tier 1 continence support, advice, guidance and signposting for nocturnal enuresis and toileting. They also offer behaviour support. This support is also provided through online workshops. The team offer tier 1 (eLearning and online presentations) and tier 2 (Face to face) awareness sessions for Anaphylaxis, Asthma and Diabetes. We also offer a follow up service for underweight children identified on the National Child Measurement Programme (NCMP), including contacting the family for a discussion and a re-measurement.

The work is hybrid, both at home and in LCC office buildings. We complete initial health assessments in clinic settings (Family Hubs and Children Centres) and/or by video or telephone calls. There is sometimes the need to undertake work in schools and patient homes.
