About the team
This is an exciting opportunity to join Hertfordshire’s Community Learning Disability Nursing service in Hertfordshire County Council working with people who live in West Herts. As a Band 6 Nurse you will work with other nursing and social care colleagues within the Adult Disability Team to reduce health inequalities for people living in the team locality.
About the role
As a Community Nurse you will work alongside paid and family carers to support people with learning disabilities to access mainstream health services by advising how to make reasonable adjustments to achieve equitable outcomes. Your skills in adapting and creating the right approach for the individual will help bridge the gaps and reduce the risk of avoidable health conditions. You will be working with social work and Occupational Health colleagues applying the Connected lives principles to empower people with learning disabilities to understand health risks and make healthy choices. As a Senior nurse you will be expected to be a Practice Assessor for Student Nurses to keep clinical standards high and ensure research-based practice.
We require people who can be creative and flexible to different approaches for different situations as no day in community learning disability nursing is the same. As the post is in the community driving is an essential requirement.
As a Band 6 Nurse you will be responsible for triaging and managing referrals to the service along with supervising and supporting junior colleagues. We welcome applications from registered nurses who are not a registered learning disability nurse if you can demonstrate experience and an understanding of the issues for this population.
About you
Essential:
- Qualified Registered Nurse
- Full UK Driving Licence and access to own car
- Some experience and knowledge in working with people with learning disabilities
- Multi-disciplinary Team working experience
- Able to cite evidence-based practice
Desirable:
- Community health care experience
- Knowledge of social care model of disability
- Teaching skills
