Are you eager to empower individuals during challenging times? We are on the lookout for passionate and resilient Community Care Officers to join our dynamic Urgent & Emergency Care Team. In this fulfilling role, you will collaborate with health and social care professionals to support individuals through crises, facilitate timely hospital discharges, and promote independence. As a Community Care Officer, your key responsibilities will include:

• Assessing and reviewing the needs of individuals and their carers.
• Coordinating personalised support and solutions.
• Connecting people with community services to enhance their independence.
• Supporting hospital discharge planning and community-based interventions.
• Collaborating with healthcare professionals and partner agencies to ensure timely and effective support.

To thrive in this role, you should possess:

• A relevant health or social care qualification (NVQ Level 3 or equivalent).
• Knowledge of community care services and legislation, including the Care Act 2014 and Mental Capacity Act 2005.
• Experience in a similar role, with at least two years in care work (paid, voluntary, or personal).
• Proficiency in using IT applications for record-keeping and reporting.
