Are you eager to help others live independently and achieve their personal goals? We are looking for a Community Care Officer to join our supportive Place Based Community Team. This role is perfect for someone who is compassionate, motivated, and ready to make a positive impact in the lives of adults, carers, and families.

As a Community Care Officer, you will:

• Conduct assessments to understand the needs of individuals and their carers.
• Develop personalised support plans that promote independence.
• Identify creative solutions and connect people with community resources.
• Manage your own caseload while collaborating with Social Workers and health colleagues.
• Contribute to safeguarding efforts and respond to urgent situations.
• Provide information, advice, and support to individuals and their families.

To succeed in this role, you should have:

• A relevant health or social care qualification (NVQ Level 3 or equivalent), or evidence of required skills and experience.
• Knowledge of community care services and legislation, including the Care Act 2014.
• At least two years of experience in care work (paid or voluntary).
• Proficiency in using IT applications for record-keeping and reporting.
