Early Intervention and Prevention Practitioner

The EIP Practitioner is a senior member of the team who supports the Coordinator in the day-to-day running of the hubs, provides oversight of the duty function, and offers guidance, coaching, and leadership to colleagues.
As a Duty Practitioner, you will often be the first point of contact for families, providing advice, guidance, and support to identify the most appropriate services to meet their needs. You will also facilitate and coordinate Team Around meetings.
Key Responsibilities
- Daily duty function within the hub, including making dynamic assessment of need.
- Act as a senior practitioner, providing coaching, guidance, and leadership to colleagues.
- Support the Coordinator with the effective operation of the hub.
- Organise staff timetables and plan service activities.
- Facilitate Team Around meetings and support plans.
- Build and maintain effective working relationships with partner agencies, including health, education, housing, and community organisations.
- Support families to access relevant local services and resources.
What Will Be Expected of Me?
You will work directly with families and young people to assess needs, plan and deliver interventions, and provide ongoing support. You will use a strengths-based, whole-family approach to empower families, promote resilience, and improve outcomes.
You will be expected to work collaboratively with colleagues and partner agencies, developing a strong understanding of local services and ensuring young people, and their families receive the right support at the right time.
Essential requirements:
- Recognised Level 5 or Degree Level qualification in a relevant subject area, or the ability to demonstrate an equivalent level of knowledge and skill gained through relevant experience and employment.
- An understanding of the risks and needs that children and young people experience.
- An understanding of the principles that support and promote the wellbeing of children, young people, and families.
- Experience of working directly with children, young people and families with complex needs and risk in a relevant setting.
- Skilled in undertaking dynamic assessments of need and risk for individuals, activities or groupwork interventions. Using assessment tools where appropriate.
