About Early Support

The Early Support Service provides targeted, family‑centred support within the Continuum Model of Support Framework, improving outcomes for young children and their families. Life can be extremely challenging for families raising a child with complex needs or a disability, and Early Support Key Workers play a vital role in making a meaningful and lasting difference.

Our team works holistically with families of children aged 0–5 years who have multiple, complex health needs and/or disabilities, ensuring the right support is in place at the right time.

  • Work in partnership with parents/carers and professionals across health, education, family services, and the voluntary sector
  • Play a key role in co‑producing, delivering and reviewing Family Support Plans, ensuring the child remains at the centre of all decision‑making
  • Liaise closely with the Early Support Coordinator to ensure personalised, effective support

You will also contribute to the delivery of the Early Support Service’s Graduated Offer, which may include supporting or coordinating community‑based groups such as Play and Stay sessions for children with additional needs and their families.

Key Responsibilities

  • Managing a caseload using the Early Help Module (EHM), including assessments, Team Around the Family (TAF) meetings and family‑centred support plans
  • Providing targeted support to families referred via Surrey C‑SPA who meet Early Support criteria
  • Coordinating multi‑agency support across health, education and the voluntary sector
  • Supporting families to understand complex medical information and care pathways
  • Supporting children’s access to appropriate early years and school provision (mainstream and specialist)
  • Contributing to the development and delivery of additional needs group support in local communities
  • Providing advice and signposting to families attending groups
  • Representing the service at local networking forums
  • Attending multi‑disciplinary meetings, including DCDT and CDT, alongside health and education colleagues
