Experienced Social Worker - Transitions Service (18-25 Years)

Are you passionate about empowering young adults to achieve independence and positive outcomes? We are seeking an Experienced Social Worker to join our Transitions Service, supporting young people aged 18-25 with disabilities, autism, learning disabilities, mental health needs and complex needs as they move into adulthood.
You will undertake strengths-based assessments, care and support planning, reviews and safeguarding work in line with the Care Act 2014, Mental Capacity Act 2005 and other relevant legislation. Working collaboratively with individuals, families, carers and partner agencies, you will promote choice, control, independence and community inclusion.
The successful candidate will have experience of managing complex cases, undertaking risk assessments, applying relevant legislation and working effectively within multi-agency settings. You will demonstrate strong analytical skills, professional curiosity and a commitment to delivering person-centred, outcome-focused practice.
