Ready to Take the Next Step in Your Social Work Career?

Are you an ambitious Social Worker looking for your next challenge? If you're passionate about making a real difference and want to develop your career in a supportive, forward-thinking organisation, we'd love to hear from you.

At South Tyneside Council, you'll join a team where Social Workers are valued, supported and empowered to thrive. We offer competitive salaries, flexible and hybrid working arrangements, manageable caseloads, and exceptional opportunities for learning, development and career progression.

Our commitment to staff wellbeing and support is reflected in our latest employee survey, where 94% of colleagues said they feel well supported at work, 95% feel their manager cares about their wellbeing, and 93% feel their manager listens to and values their views. In addition, 96% of colleagues said their manager demonstrates our PROUD values, helping to create a positive and inclusive working environment where people can flourish.

Whether you're looking to expand your skills, work towards additional qualifications, or progress into specialist and leadership roles, we'll invest in your future through a comprehensive learning and development programme, including support to further your post-qualifying development. As one colleague recently reflected, "South Tyneside Council offers something for everyone in terms of support and training."

About the Role
This is an exciting opportunity to join our Hospital Discharge Team, playing a vital role in supporting adults with care and support needs to achieve the outcomes that matter most to them.

Working in a fast-paced and rewarding environment, you will undertake a range of assessments and interventions, working closely with individuals, families, carers, health colleagues and partner organisations to support safe, timely and person-centred hospital discharges.

You'll be at the heart of delivering our Home First and Discharge to Assess approach, ensuring people spend no longer in hospital than necessary and are supported to recover, regain independence and remain connected to their communities. Rather than making long-term decisions in a hospital setting, you'll help ensure assessments take place in the most appropriate environment, focusing on people's strengths, what matters to them, and the support available within their families and communities.

As part of a multidisciplinary team, you will work closely with NHS and partner agencies to facilitate seamless transitions from hospital, reduce delays, promote independence and improve outcomes for local people. This is an opportunity to make a real difference every day by helping people return to the place they call home and supporting them to live as independently as possible.

South Tyneside Council is committed to innovation, partnership working and continuous improvement. We are leading the way in developing strengths-based, person-centred approaches that focus on prevention, recovery and reducing long-term dependency on services.

The successful applicants will share our enthusiasm for improving outcomes for people, contributing to service development and influencing performance at an individual, team and service level.

In return, you'll benefit from a wide range of tailored Continuing Professional Development opportunities, alongside ongoing support to help you achieve your career aspirations.

About You
We're looking for enthusiastic and motivated Social Workers who are committed to delivering high-quality, person-centred practice.
You will:
  • Be a qualified Social Worker registered with Social Work England.
  • Have experience of working within Adult Social Care and a sound understanding of relevant legislation, hospital discharge guidance and best practice.
  • Be passionate about promoting independence, wellbeing, choice and control.
  • Have excellent communication, assessment and partnership-working skills.
  • Be committed to your own professional development and continuous learning.
  • Most importantly, you'll share our ambition to support people to live the best lives possible and be excited by the opportunity to contribute to service innovation and improvement.
