Job Title: Health & Care Programme and Development Manager – Intelligence and Impact
Salary: £53,460 - £61,638 per annum
Grade: G
Contract Type: 2-year Fixed Term Contract
Working Hours: 37 Hours per Week
About the Department
The service is responsible for the development and implementation of the Adult Social Care transformation programme - as part of Better Care Together Thurrock and the Integrated Care Strategy. Although the role will support a range of transformation programmes, there will be a focus on intelligence-led system redesign and evaluation frameworks across the health and care partnership.
About the Role
The role sits within Adult Social Care but will work in partnership with Housing and local NHS and voluntary and community sector partners.
As a Health & Care Programme and Development Manager, you will be responsible for:
- The development and implementation of intelligence-led system redesign and evaluation frameworks across the health and care partnership (Better Care Together Thurrock)
- The development and implementation of our approach to place-based integration
- Improve the outcomes of local people through the development of integrated and person-focused solutions and system-change
This role offers the chance to make a real difference by ensuring place based intelligence support effective decisions, improve services, and allocate resources more effectively to improve service quality, efficiency, workforce practices, and outcomes for individuals.
What We’re Looking For
We’re looking for someone who has:
- A good understanding of health and social care.
- A good understanding of the role of the community and voluntary sector and asset-based community development.
- Experience of delivering change initiatives and/or system redesign to improve health and care outcomes.
- The ability to and experience of working in partnership with others to develop and deliver change.
DBS Check: A Basic Disclosure and Barring Service (DBS) check is required
