Make a difference from the very first conversation. If you are passionate about Social Care and helping people live as independently as possible, this is an exciting opportunity to join a supportive, forward-thinking team and build a rewarding career in Adult Social Care.

Care Direct Plus is part of the front door to Adult Social Care in Devon. Our Assessment and Review Team supports adults and unpaid carers who contact us for information, advice and help. As a Social Care Assessor, you will listen to what matters to each person, recognise their strengths, and work with them to identify practical, person-centered solutions.

The role is varied and meaningful. You will support people with disabilities, older people and unpaid carers, mainly by telephone, while working closely with families, health colleagues, housing, voluntary organisations, care providers, and other professional partners.

What you will do

• Have purposeful conversations with people about their needs, strengths, goals, and the outcomes they want to achieve.
• Provide clear information, advice, and signposting to help people remain independent.
• Assess risk, make appropriate referrals, and review of care and support arrangements.
• Work collaboratively with individuals, carers, professionals, and partner organisations.
• Complete holistic Care Act assessments and develop person-centered support plans.
• Work focused on prevention and short-term services to continue to prevent, reduce and delay the need for long term care.
• Keep accurate and timely records using our digital systems.

What we are looking for

We would particularly welcome applications from people with experience in social care, health, community support, or a related setting.

You will also need:

• A compassionate, curious and strengths-based approach.
• Confidence communicating with people who may be experiencing difficult or challenging circumstances.
• Clear written and verbal communication skills.
• The ability to organise and prioritise a busy workload while contributing positively to a team.
• Good IT skills and a willingness to develop your confidence with digital tools, including AI.
• A strong commitment to equality, diversity, inclusion, and excellent service.

What you can achieve

A thorough induction, training, and reflective supervision will be provided to support you to develop your confidence in assessment, risk evaluation, support planning and reviewing care arrangements. You will learn more about safeguarding, the Care Act 2014 and the wider social care system, while building valuable relationships across health, housing, voluntary and care services.

You will be supported to grow in the role, contribute ideas, and help shape how adult social care is delivered. Most importantly, your work will help people retain independence, regain control, and achieve outcomes that matter to them. We have a vibrant and happy team that is welcoming and experienced in their roles. They are excited to support new team members to join the centre!
