• Complete Occupational Therapy assessments and interventions.
• Undertake Care Act assessments and reviews.
• Recommend equipment and adaptations to promote independence.
• Support complex case management, safeguarding enquiries and multi-agency working.
• Contribute to Community DoLS and Court of Protection work where required.
• Work closely with health colleagues, care providers, housing teams and voluntary sector partners.
• Help reduce delays in care, prevent unnecessary hospital admissions and support timely discharge from hospital.

About You

• Hold a degree or diploma in Occupational Therapy.
• Are registered with the HCPC.
• Have experience of working with adults who have complex physical, functional or social care needs.
• Demonstrate sound clinical reasoning and risk assessment skills.
• Have excellent communication and partnership-working abilities.
• Are passionate about promoting independence, wellbeing and person-centred practice.
• Can work autonomously whilst contributing positively to a multidisciplinary team.

Community experience is desirable but not essential for the right candidate.
