We are seeking a motivated and enthusiastic Occupational Therapy Assistant to join our Reablement Care at Home Service. This is an exciting opportunity to support people to regain, maintain and maximise their independence within their own homes following illness, injury, hospital admission, or a change in circumstances. Working as part of a multidisciplinary team, you will support Occupational Therapists in delivering person-centred interventions that promote independence, wellbeing, and positive outcomes for individuals across our communities. The role will involve working directly with service users and carers to implement reablement plans, monitor progress, and support individuals to achieve their personal goals. The successful candidate will have experience working in health, social care, rehabilitation, reablement or a related setting. Be able to demonstrate an understanding of person-centred practice and promoting independence. Possess excellent communication and interpersonal skills and be able to work independently and as part of a team. Have good organisational and time management skills and be able to maintain accurate and timely records.
