Outreach Support Worker – Empower People to Live Well and Independently

We’re looking for an Outreach Support Worker to join our team in Essex, providing vital support to vulnerable people living independently in the community. Covering the Mid and North Essex area, your work will help prevent homelessness, reduce the need for adult social care, and empower people to thrive.

What your day might look like

This is not a traditional care role as it’s about empowering people, not doing things for them. You’ll help customers build resilience, learn new skills and take control of their future.

  • Visiting customers in their homes to provide planned support or crisis intervention.
  • Supporting individuals to maintain their tenancies, understand their rights and responsibilities, and navigate housing-related issues.
  • Helping customers access benefits, training, education, health services and community opportunities.
  • Encouraging people to build independent living skills, from budgeting and daily planning to managing appointments and developing routines.
  • Working in partnership with social care, health providers, voluntary agencies and community organisations to coordinate support.
  • Completing assessments, triaging referrals and managing your own caseload.

Key responsibilities

  • Provide pre-tenancy guidance and ongoing tenancy support.
  • Help customers develop the skills needed to manage their tenancy and navigate challenging situations.
  • Deliver information, advice, guidance and training to build independent living skills.
  • Manage referrals, undertake assessments, and provide crisis support where required.
  • Hold a caseload of customers with varying support needs, including those with learning disabilities, mental health needs, physical disabilities, or age-related challenges.
  • Encourage and support customers to engage in activities, forums and community involvement programmes.

We’re looking for someone who:

  • Is Ambitious, Caring, Collaborative, Empowering, Trusted.
  • Has experience (professional or voluntary) in a supportive role
  • Has worked with individuals facing mental health, health-related, or employment challenges
  • Understands housing support needs and welfare benefits
  • Can provide homeless support and ongoing tenancy support
  • Communicates clearly and solves problems with confidence
  • Understands safeguarding procedures and is able to safeguard vulnerable people.
  • Is organised with good time management and IT skills.

Please note - This role will require a Basic DBS check and full driving licence with access to your own vehicle
