About The Role

We are recruiting for a Personal Advisor to join our Throughcare Service .
If you are passionate about supporting care experienced young people to acquire appropriate independent living skills and to successfully transition to independent living arrangements, we would love to hear from you.
In this role, you will:
- Manage a caseload and complete, monitor and regularly review holistic Pathway Assessments and Plans with young people, carers and relevant professionals, ensuring strengths, needs, views and actions towards independence are clearly recorded.
- Respond to requests for support, assess social care and accommodation needs, and coordinate appropriate advice, services and interventions across health, education, training, employment, finance, youth justice, immigration, relationships, personal support and practical skills.
- Work collaboratively with young people, families, carers, specialists and partner agencies, arranging translation, interpretation or advocacy where required and identifying service gaps for management attention.
- Maintain accurate and timely case records, calendars and statutory processes; work within Council policies, financial regulations and budget limits; support duty and emergency cover, flexible working, professional development and an agreed lead area of practice.
About You
We are looking for someone who can demonstrate:- Knowledge of childcare legislation, guidance and regulations, including the Children Act 1989 and the Local Authority’s duties towards children looked after, care leavers and unaccompanied asylum-seeking children.
- Understanding of care leavers’ transition to independence, including education, benefits, accommodation, employment, housing policy and meaningful participation in service design.
- Have strong organisational and communication skills, including meeting deadlines, statutory report writing, accurate case recording and building effective partnerships that improve outcomes.
- Commitment to equality, diversity, continuous professional development and sharing good practice, with flexibility to work occasional evenings and undertake the required DBS checks
