Following a comprehensive review of our SEND service, we have introduced a number of new roles as part of our wider SEND Transformation. These changes represent a significant investment in our workforce and reaffirm our commitment to improving outcomes for children and young people with SEND. By creating a stronger and more sustainable service structure, we are enhancing our ability to respond to increasing demand, deliver high-quality support, and work effectively with families, schools and partners.

This recruitment campaign offers an exciting opportunity to join a service that is ambitious about improvement, committed to inclusion, and focused on ensuring that every child and young person with SEND has the opportunity to succeed and flourish.

Do you want to help children and young people overcome barriers to education and achieve their potential?

We are looking for a motivated and compassionate Reintegration Practitioner to join our growing SEND and Inclusion Service.

This is a rewarding opportunity to make a real difference to the lives of children and young people with Special Educational Needs and Disabilities (SEND) who are struggling to access education. Whether barriers arise from anxiety, emotionally based school avoidance, attendance challenges or difficulties within a current placement, you will play a vital role in helping children and young people reconnect with learning and achieve positive outcomes.

No two days will be the same. You will work directly with children, young people and families, building trusting relationships, identifying strengths, and creating personalised plans that support successful reintegration into education. Your work will help children regain confidence, develop resilience and access the opportunities they deserve.

Working alongside schools, settings and partner agencies, you will be part of a passionate and dedicated team committed to inclusion, early intervention and ensuring every child has the best possible chance to thrive.

About the Role

  • Provide tailored support to children and young people with SEND who are experiencing barriers to education.
  • Work directly with children and young people to build confidence, resilience and engagement with learning.
  • Develop and deliver personalised reintegration and progression plans.
  • Support attendance recovery and successful returns to education, including phased reintegration programmes where appropriate.
  • Work closely with schools, educational settings and alternative provision to promote positive outcomes.
  • Build strong relationships with families and provide practical support throughout the reintegration process.
  • Help prevent placement breakdown by identifying concerns early and implementing effective interventions.
  • Attend and contribute to multi-agency meetings and reviews.
  • Monitor progress and outcomes, adapting support plans where required.
  • Maintain accurate records and contribute to service development and continuous improvement.

  • Ability to develop and implement practical, child-centred support plans.
  • Experience of working collaboratively with families, schools and multi-agency partners.
  • Strong communication, relationship-building and problem-solving skills.
  • Ability to manage competing priorities and maintain accurate records.
  • Commitment to promoting inclusion and improving outcomes for children and young people.
