Senior Social Worker - Mental Health Transitions Team (Ages 16-25)

37 Hours per Week | Adult Social Care | Wakefield

Are you passionate about making a lasting difference in the lives of young people?

We are looking for an experienced and enthusiastic Senior Social Worker to join our Mental Health Transitions Team, supporting young people aged 16-25 as they move from children's services into adulthood.

This is more than a social work role. It is an opportunity to help shape a young person's future at one of the most important and often challenging times in their life. You'll work alongside young people, families, partners and professionals to ensure transitions are planned, supported and focused on what matters most to the individual.

If you are someone who values relationship-based practice, partnership working and empowering young people to achieve their goals, we'd love to hear from you.

What You'll Be Doing

As part of our Adult Mental Health Social Work Service, you will:

  • Support young people from the age of 16 as they prepare for and move into adult mental health services.
  • Build trusting relationships that provide continuity, stability and reassurance during times of change.
  • Work closely with partners across CAMHS, Children's Services, SWYPT, education providers and ICB colleagues to create joined-up and seamless pathways.
  • Champion strengths-based, trauma-informed and recovery-focused approaches.
  • Help young people understand their options, have their voices heard and take an active role in shaping their futures.
  • Contribute to the ongoing development of innovative transition pathways across Wakefield.

Who We're Looking For

We're seeking a motivated and experienced Senior Social Worker who:

  • Has a strong foundation in statutory social work practice and experience within mental health, transitions or related services.
  • Is passionate about supporting young people and understands the unique challenges they can face during transition into adulthood.
  • Demonstrates excellent strengths-based and person-centred practice.
  • Has a sound understanding of relevant legislation, including the Care Act 2014, Mental Health Act 1983, Mental Capacity Act 2005, Human Rights legislation and children's legislation.
  • Enjoys working collaboratively and building positive relationships across services.
  • Brings creativity, curiosity and a commitment to achieving the best possible outcomes for young people.
  • Shares our belief in promoting independence, choice, rights and positive risk-taking.

Why Join Us?

At Wakefield, we're committed to creating an environment where talented social workers can thrive.

We offer:

  • The opportunity to be part of an ambitious and evolving service making a real difference to young people's lives.
  • A supportive and experienced team that values collaboration, innovation and learning.
  • Excellent professional development opportunities and access to tailored training.
  • Regular supervision and support to help you grow in your career.
  • Flexible and hybrid working arrangements that support a healthy work-life balance.
  • A competitive salary with incremental progression.
  • Generous annual leave entitlement.
  • An excellent occupational pension scheme.
  • The opportunity to influence and shape developing practice within Mental Health Transitions.

Join us and help ensure that every young person experiences a positive, supported transition into adulthood.

What can I expect?

  • A competitive salary
  • Generous holiday entitlement with the option to buy up to 10 extra days of annual leave
  • Hybrid working (subject to role suitability)
  • Access to high-quality training programs to advance your career to the next level.
  • We are committed to supporting the wellbeing of our staff. We recognise that resilient teams need good work life balance as well as supportive leadership.
  • Discounted and subsidised public transport (Arriva Bus Annual Travel Ticket, Northern Trains Limited (NTL), MetroCard).
