About the Role:

• We are looking to recruit enthusiastic and hardworking, post qualifying social worker within our Transition Team in the Wakefield district.
• You will work closely with our key partners in the children’s teams and the CTLD health team, to carry out person led assessments with children and adults from the age of 16 years who are transitioning from children’s services into adult social care.

• The team consists of 5.5 social workers. As a social worker on the team, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am – 5pm and to 4.30pm on a Friday.

What will be expected from me?

• As a team player, you will need to demonstrate an ability to, work under pressure, be well organised and able to effectively prioritise and risk assess/manage a complex caseload. You will have experience, knowledge and commitment to strength-based assessments and support planning. You will be able to undertake complex tasks such as chairing significant meetings, offering expert support and advice to case conferences, producing high quality and more specialised assessments and reports for a range of functions including court.

Essential requirements:

• Applicants for this role must be qualified to a degree level or equivalent and be registered as a social worker with Social Work England, with essential post qualifying relevant training and experience.
• The ability to travel and access to your own vehicle is essential.
