Social Care Practitioner – PACT Team

We have an excellent opportunity available for two social care practitioners to join our Adult Social Care Team on a Full Time, Permanent Basis. This role part of our new People Achieving Change Together (PACT) team.

The PACT Team will work alongside adults who may not feel able to engage with traditional services. This includes young people transitioning into adulthood, people experiencing complex or high-risk circumstances, those at risk of exploitation, and individuals whose hoarding is creating serious safety concerns. The team will hold a small caseload, allowing time to build meaningful, trusting relationships and to respond quickly and flexibly when someone is at risk or in crisis. We will work with adults who have Care Act eligible needs, or who may reach that point without early, compassionate, and relational support.

As social care practitioner, you’ll spend your time getting to know people, building trust and finding practical ways to help them move forward. You’ll work alongside individuals, families and other services to manage risks, overcome barriers and help people feel safer, more independent and in control of their lives. No two days will be the same.

Key responsibilities:

Working under the supervision of a registered practitioner, you’ll:

  • build trusting relationships with people who may be reluctant to engage with services
  • support assessments, reviews and support planning, focusing on people’s strengths, wishes and goals
  • provide practical and motivational one-to-one support, including acting as a “declutter buddy” where appropriate
  • help people access services, benefits, community support and technology that could improve their independence
  • identify and manage risks relating to hoarding, self-neglect, exploitation and safeguarding
  • work alongside housing, health, community safety, education, voluntary organisations and other partners
  • support prevention and early intervention, helping people stay safe and independent in their community.

Candidate requirements:

You don’t need to know everything from day one. What matters is that you can build relationships, communicate well and take a thoughtful and person-centred approach.

You’ll need:

  • experience of working in health or social care, or in a customer-facing role
  • experience of working with vulnerable adults
  • GCSE Maths and English, or equivalent
  • an understanding of adult social care legislation and safeguarding
  • good written and verbal communication skills
  • confidence using IT and the ability to learn social care recording systems
  • a commitment to relationship-based, trauma-informed and strengths-based practice
  • the ability to remain professional when working under pressure
  • a full EU driving licence and access to a car for daily use
  • Experience of working with people who need different approaches to engagement would be an advantage, alongside an understanding of hoarding, self-neglect, trauma and the signs of exploitation.
