We are recruiting a Social Worker at Caerphilly County Borough Council.
Working hours: 37 Hours (7.24 Monday – Friday)

We pay an attractive salary of £39,152 - £41,771 and offer access to training and development opportunities.
We’re looking for a dynamic Social Worker to join our fast‑paced Hospital Discharge Service within the CRT. Working primarily from Ysbyty Aneurin Bevan (YAB) in Ebbw Vale, you’ll play a key role in ensuring safe, timely, person‑centred discharges and building strong connections with community support services.
You’ll work closely with nurses, therapists, and wider health and social care teams to create smooth pathways home for residents. While YAB will be your main base, flexibility to support other sites (e.g., NHH, PCH) may be required. You’ll be backed by an experienced, supportive team across Caerphilly and Blaenau Gwent.

You must possess a Diploma/Degree in Social Work or predecessor and be registered as a Social Worker with Social Care Wales (or able to transfer from another recognised body).
The current registration requirements state that completion of the Consolidation of Practice qualification within your first 3 years of practice is mandatory (if you qualified after 1st April 2016). Failure to successfully complete this qualification will result in you being unable to renew your Social Work Registration or practice as a Social Worker.
