Our Older Persons & Physical Disability Service is looking for a passionate and dedicated Social Worker to join our amazing team and help make a difference today.

About the Role:

As a Social Worker within Southwark’s Adult Social Care service, you will work flexibly and creatively with adults, their families, and wider professional networks to promote independence, choice and wellbeing. Using a strengths‑based approach, you will carry out assessments, care planning and reviews, while supporting people to achieve their personal goals and overcome barriers.

You will collaborate closely with partner agencies and multi‑disciplinary teams, ensuring statutory duties are met while delivering person‑centred, outcome‑focused practice.

About You:

We are looking for motivated and skilled Social Workers who share our values and commitment to high‑quality practice. You will bring:

  • Experience of working with people with disabilities and/or mental illness

  • A strong understanding of the Care Act, Mental Capacity Act, and Adult Safeguarding

  • Evidence of continuing professional development

  • Up‑to‑date knowledge of changes in legislation and social work practice

  • The ability to advance human rights, social justice, and economic wellbeing

  • High‑quality written and verbal communication skills, including the ability to analyse complex information and produce clear, accurate records

  • Experience of working within multi‑disciplinary and multi‑agency settings

  • A collaborative, reflective and proactive approach to professional practice.

This post requires a satisfactory Disclosure and Barring Service (DBS) check at Enhanced - Adults' and Children's (Barred) level.

Additional Information:

    Hours of work, 36 hours per week Monday - Friday.
