We are seeking dynamic and compassionate Social Workers to join our Urgent & Emergency Care Team in Mid Nottinghamshire. This role is vital in supporting adults through hospital discharge and recovery in the Mansfield, Ashfield, and Newark areas. As a Social Worker, you will:

• Collaborate within an integrated multidisciplinary team at the Transfer of Care Hub.
• Make informed discharge pathway decisions for individuals needing support.
• Conduct Care Act assessments, Mental Capacity Act assessments, and Safeguarding enquiries.
• Apply critical thinking and professional judgement to manage risks effectively.
• Foster independence, wellbeing, choice, and control for adults requiring support.
• Build positive relationships with individuals, families, and professionals to achieve person-centred outcomes.
• Manage a varied caseload in a fast-paced environment while upholding dignity and human rights.

To succeed in the role you will need:

• A recognised Social Work qualification and registration with Social Work England.
• Knowledge of adult social care legislation and relevant policies.
• Experience in adult safeguarding and partnership working with health and external agencies.
• Strong organisational skills to manage a mixed caseload effectively.
