We are looking for a social worker with experience of working in Continuing Health Care services to join the Complex CHC Team.
The role is for a part time Social Worker,18.5 hours ( working days must include a Thursday)
As a Social Worker you will be supported by our team’s Principal Care Manager to understand the role and be provided with continuous professional development.
The role is a fast-paced role, and you will work directly with individuals, nurse assessors and other health professionals , care providers, and other stakeholders.
You will be attending in person meetings in the community , working remotely and in the office for duty work. N.B. All IT equipment will be provided.

You will be working in line with key legislation such as the Care Act 2014 and frameworks such as the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care 2022.
You will be attending multi-disciplinary meetings to represent Adult Social Care to discuss health needs and make decisions on behalf of the local authority.
You will be gathering information about the person using the 3 conversations model of approach , which is person centred and strengths based.
You will ensure the person’s voice is throughout all your work with them .
You will be observant, actively listen and apply professional curiosity to your practice.
You will be able to identify the need to apply safeguarding and to raise concerns appropriately.

You will need to have:
- Experience of working with a range of individuals
- Experience of working collaboratively with partner organisations
- Experience of applying the National Framework for NHS (2022)
- Knowledge of the Care Act 2014 and other key legislation
- Experience of completing assessments
- Effective Communication and IT skills
- Experience of safeguarding
- Ability to prioritise and effective time management skills
- Support individuals with a strength-based approach
- Have a good understanding of risk management
- Ability to work to deadlines

Salary range
- £42,839 - £46,142 per year
