The Role
We are seeking a motivated and experienced Social Worker to join our Adult Social Care Transitions Team.
Our team supports young people aged 16–25 with a wide range of needs as they move from children’s services into adulthood. This is a rewarding opportunity to work alongside young people, families, and partner agencies to deliver timely, person-centred transition planning that promotes independence and achieves positive outcomes.
You will manage a varied and complex caseload, applying your professional judgement within a supportive team environment, and contributing to high-quality, strengths-based practice across the service.
Key Responsibilities:
- Undertake Care Act assessments, reviews, and support planning in line with statutory guidance
- Apply the Mental Capacity Act and contribute to Best Interests decision-making
- Complete safeguarding enquiries under Section 42 of the Care Act
- Prepare clear, evidence-based reports to inform professional decision-making
- Work collaboratively with individuals, families, health colleagues, providers, and partner agencies to deliver coordinated responses to complex needs
- Contribute and formulate transition plans, including participation in EHCP reviews
- Work with Brokerage and providers to source appropriate care and support
- Monitor support arrangements and escalate concerns relating to quality, risk, or provider sustainability
- Maintain accurate, timely, and defensible case records
- Ensure compliance with all relevant policies, procedures, and statutory frameworks
- Complete Community DoLS applications where appropriate
- Manage a defined caseload, prioritising work effectively and working with autonomy
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.
This position is currently based at our Castlewood offices in Clevedon. Please be advised that this office will be closing in 2027. Consultation is currently underway to determine the new location for colleagues based in Castlewood. This will be at Weston-Super-Mare or within North Somerset.
Work Style
At North Somerset Council, we offer a hybrid working environment based on four distinct work styles. Depending on the role and personal circumstances, you will fall into one of these categories. However, please note that during the induction period, it may be necessary to spend more time in the office to support your learning and development. We are flexible and open to discussions about your preferred working style, and if you wish to be in the office more frequently, this can be negotiated.
This role falls into the category of Mobile/Field Worker which means you can expect to work in the following way:
Mobile/Field Worker – Remove heading
- 0-1 day in the office
- Includes field workers who primarily work remotely.
- Comes into the office for meetings or focused work. Virtual connections with the team are essential, with portable technology supporting work on the go.
About You
We are looking for someone who is passionate about supporting young people into adulthood and can demonstrate:
Essential Criteria
Qualified Social Worker registered with Social Work England
Successful completion of ASYE
Experience of working in statutory adult social care
Ability to manage a complex caseload and make sound, defensible decisions
Strong working knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding frameworks
Experience of effective multi-agency working
Strong communication skills and ability to produce high-quality written work
Ability to work autonomously, prioritise workload, and meet deadlines
Desirable Criteria
Experience of transitions work (16–25)
Understanding of EHCP and SEND processes
Experience of working with people with learning disabilities and/or complex needs
Experience of working with providers and Brokerage
Knowledge of CHC processes
Experience of supporting or mentoring others or contributing to service development
