As a Targeted Practitioner, you will work flexibly across our services, adapting to meet the needs of disabled children and young people throughout the county. You will play a vital role in supporting our leadership team and providing bespoke assistance to children and their families. This is an exciting opportunity to make a real impact in your community!
In this role, you will:
- Deliver targeted interventions and support for disabled children and young people.
- Collaborate with co-ordinators to ensure the smooth running of our countywide service.
- Provide direct, tailored support to children and their families, helping them navigate challenges and access resources.
- Engage with various stakeholders to promote the well-being of children and families.
To succeed as a Targeted Practitioner, you should have:
- A minimum of 2 years’ experience in Children’s Social Care, Early Help Services, or Customer Services, with a focus on direct service delivery.
- A good general education, ideally to NVQ level 3 or equivalent experience.
- Knowledge of relevant legislation and guidance affecting children and families.
- A full driving licence (unless registered disabled).

- Exhibit strong interpersonal skills, enabling effective communication with children, families, and colleagues.
- Show a willingness to work flexibly, including outside core hours when necessary.
