About the team

An exciting opportunity has arisen within Hertfordshire County Council’s Transfer of Care Hub (TOCH), a fast-paced, multi-disciplinary service delivering a coordinated and integrated approach to hospital discharge across multiple acute hospital sites and community settings.

The Transfer of Care Hub plays a critical role within the wider health and social care system, working in close partnership with NHS colleagues and other stakeholders to support safe, timely and effective discharges. Covering several hospital sites across Hertfordshire, as well as out-of-county placements, the service manages complex pathways and system demand to ensure individuals experience seamless transitions from hospital into the community.

About the role

As Team Manager, you will provide strong operational leadership within a dynamic environment, ensuring high-quality, person-centred practice alongside robust performance management to support system flow and improved outcomes for residents. You will contribute to the effective management of performance, provide professional oversight, and support service development within a fast-paced and evolving service.

The successful applicant will demonstrate sound knowledge of safeguarding practice, experience working with people with dementia, and a strong understanding of the personalisation agenda and Individual Budgets. You will ensure individuals and their families remain central to all decision-making, delivering strengths-based support within the legislative framework.

We are a busy service covering a large and diverse geographical area and are seeking a motivated and experienced leader who can bring energy, innovation and a clear focus on performance, quality and continuous improvement.

The Transfer of Care Hub operates a seven-day hospital discharge service (08:00–18:00, including weekends). The post holder will participate in a service-wide rota, working one full weekend every eight weeks, with time off in lieu provided.

Key responsibilities include:

  • Leading and overseeing day-to-day practice within the team, ensuring compliance with statutory duties, policies and procedures, and embedding a strong performance management culture.
  • Managing team performance to ensure the timely and effective completion of hospital discharges, reviews and assessments, supporting system pressures across multiple hospital sites.
  • Inspiring, developing and managing a team of Social Workers and Community Care Officers to deliver innovative, responsive and person-centred care.
  • Building and maintaining strong working relationships with acute hospitals, community services and wider partners to support integrated working across TOCH.
  • Ensuring effective management of resources and budgets, alongside quality assurance and service improvement activity.
  • Acting as the lead manager for complex and high-profile safeguarding enquiries and serious concerns.

About you

Essential:

  • Registered Social Worker
  • Full driving licence
  • Significant experience of managing teams within a busy, high-demand environment
  • Strong leadership skills with a proven ability to deliver performance and manage competing priorities
