Are you passionate about helping people maintain their independence and live safely in their own homes?

Following significant investment into our Occupational Therapy service, we are looking for a motivated and compassionate Therapy Assistant Practitioner (TAP) to join our innovative Right Size Care Team. This is an exciting opportunity to support adults with care and support needs through strengths-based assessments, personalised goal planning and practical interventions that promote independence, wellbeing and quality of life.

Working alongside Occupational Therapists, health professionals, care providers and community partners, you will help deliver timely, person-centred solutions that reduce, delay and prevent the need for ongoing care and support.

About the Role

As a Therapy Assistant Practitioner, you will:

  • Complete needs-based assessments and functional assessments within people's own homes.
  • Support individuals to identify personal goals, strengths and opportunities to maximise independence.
  • Prescribe and review community equipment and minor adaptations under Occupational Therapy guidance.
  • Develop and review goal plans using strengths-based approaches.
  • Contribute to moving and handling assessments and support the implementation of proportionate care.
  • Work closely with multidisciplinary teams, voluntary organisations and health partners.
  • Maintain accurate electronic records and contribute to service development.
  • Provide advice, guidance and support regarding equipment, adaptations and reablement approaches.

About You

We are looking for someone who can demonstrate:

Essential Requirements

  • NVQ Level 3 or 4 (or equivalent qualification).
  • Five GCSEs or equivalent qualifications.
  • Experience working within health, social care or a related setting.
  • Experience supporting people with disabilities and care and support needs.
  • Knowledge of safeguarding adults and relevant legislation.
  • Understanding of moving and handling principles.
  • Knowledge of assistive technology, disability equipment and adaptations.
  • Strong communication, organisational and prioritisation skills.
  • Ability to work effectively within a multidisciplinary team.
  • Commitment to person-centred practice and promoting independence.

Desirable

  • Trusted Assessor Training or Advanced Award in the Provision of Community Equipment.
  • Experience of case management with supervision.
  • Knowledge of housing adaptations, grants and social housing policy.
  • Experience of personalised goal planning.
