The objective is to enhance their independence through a strengths-based methodology and a person-centred approach.<br><br></p>\n<p>You will hold a varied caseload and will be expected to work independently and to interpret and analyse varied information or situations and to produce solutions.</p>\n<p><br>You might evaluate and suggest basic equipment or TEC, support goal planning, offer short-term care or enablement, and work closely with care providers, Housing Services, healthcare peers, and the voluntary and community sector.</p>\n<p> </p>\n<p>You will also support the team’s Duty function triaging cases that come into the team with support from Occupational Therapists. <br><br>You will work within the legal framework provided by the statute, guidance, policies, and procedures.<br><br>Role requirements</p>\n<ul>\n<li>Experience working with people</li>\n<li>Experience working within the Health or Adult Social Care</li>\n</ul>\n<p><br>We offer excellent support, training, and induction.
