Do you want to be one of our Community Care Officers in the AMAZING Community Hospitals and Transfer of Care team? Lincolnshire County Council is looking to appoint a full time Level 2 CCO to our team based in Louth.
You will need to be creative and passionate about supporting people in a person centred way and be able to work in a multi-disciplinary team, taking responsibility for an allocated caseload under the direction of a line manager/supervisor. You will have the ability to work on your own initiative and to reflect on your practice, ensuring continuous professional development, along with good verbal and written communication skills.
