Other duties include: </p>\n<ul>\n<li>Carry out care home and home visits and consult with service users, carers and relevant agencies and professionals.</li>\n<li>Ensure reviews, assessments and support plans are carried out in a person centered and outcome focused way.</li>\n<li>Commission and make arrangements for the provision of new services when needed or refer onto other appropriate agencies.<br><br>\n</li>\n</ul>\n<p>The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
